First-time tip + chat-bubble cursor

Only show the "click to leave a comment" cursor note before the
first comment. All subsequent comments are made with a simple cursor
icon that looks like a chat bubble with a plus inside.

Bubble-wrapped comment count

The numeral indicating open comments is placed inside a matching
chat bubble icon (outline).
